Form 10784 Activity Report
NEW FAITH IN ACTION FRATERNAL PROGRAMS REPORT FORM (10784)
Following the launch of the Faith in Action program model, a new form has been created that will allow reporting for most of the Supreme-recommended programs. This new form will replace 24 individual report forms, streamlining the number of forms council officers are expected to use. To fill it out electronically, you will need to enter your membership number as well as your last name. Once the electronic form is submitted, you will get a copy of the form emailed back to you that will look like the one below. The electronic form is available at the Knights of Columbus website. Foe paper copies ask the Grand Knight.
